download and print the rules and regulations. They will instruct you on the
steps involved in completing your application.

Here is the application form. You can fill it out
online and then print it out or print it blank and fill it out by hand. Either
way, it must be signed and then mailed back to CSA.
Scholarship Program Application
Applicants wishing to renew their scholarship need only to fill out this form. But you must also arrange for a transcript of your grades sent to CSA for the entire year which you were awarded a scholarship.

This is the form that you need to print out and mail
to your lodge secretary. If you don’t know who that is or don’t have that
address, you can look in the Lodge Directory in the May Journal, available
online.
